Get every tab as a CSV by email, then import them into your own Google Sheet. The previews below are open to everyone and use fictional example data.
CSV files carry the layout and example rows only, not formatting, charts, checkboxes or a shared Google Form. Formula cells arrive as text: check each one, then delete the leading apostrophe to switch it on.

Set targets together. Example frequencies are planning inputs, not health recommendations.

Daily Grid
After importing, turn B:H into checkboxes. COUNTIF counts completed days; the consecutive streak uses the previous day, not the weekly count.

Frequently asked questions
How do I add checkboxes?
Import the Daily Grid CSV, select the seven day columns and choose Insert → Checkbox in Google Sheets. Check the imported TRUE/FALSE values.
What does weekly adherence mean?
Completed days divided by the agreed weekly target, capped at 100%. It is different from a consecutive-day streak.
Can I change the habits?
Yes. Change labels and targets to match the client. Keep a separate file for each person and avoid collecting unnecessary health details.
