Before collecting responses

What is a PAR-Q?

A physical activity readiness questionnaire supports pre-participation screening. Use the PAR-Q+ Collaboration’s official forms and follow their instructions. This page supplies an intake/status structure, not the questionnaire.

Is a PAR-Q a legal document?

Requirements vary by country, state, insurer and service. Ask your insurer or professional body what screening and records you need; do not assume this template satisfies legal duties.

Can I complete it online?

Use the official publisher’s current options and instructions. You can build your own intake form from these fields, but no Google Form or copy link is supplied here.

How long should I keep it?

Set a documented retention policy with your insurer and an appropriate adviser. Keep information only for the purpose and period you can justify; this template does not set a universal retention period.
